The John and Matthew Gaudreau Foundation (JMF) honors the enduring legacy of John and Matty by supporting projects and initiatives that align with the values and passions that they championed during their lives. At the heart of our mission is a commitment to serving hockey families that have faced tragedy and families that have been affected by drunk or reckless driving, like ours. It is our hope that this grant opportunity will help cover costs for families navigating unspeakable challenges.  Before you complete this grant application form to apply for support, please read through the eligibility requirements and application process.

Eligibility Requirements 

•    Must be located in the United States or Canada.
•    Must be a family that has at least one member on a hockey team (youth, college, or pro) or be a family impacted by drunk or reckless driving. 
•    In need of financial support to pay for a critical unmet need.

APPLICATION PROCESS​

There are three simple steps:

1. Complete and submit Grant Application to info@johnandmatty.org. 

2. JMF staff will review your application and contact you to let you know if we need more information. 

3. A member of the team will then contact you to let you know the outcome of the review. If a grant is awarded, our team will make arrangements for payment directly to the organization.

Please also note the following: 

• JMF may announce recipients publicly (e.g., in a press release).JM

• JMF staff may request pictures and testimonials from your program to include in marketing and reports.

• A member of the JMF team will contact you 6 months after the initial grant to assess the impact that the grant has made on your program.

• Please share our organization with programs you think would be eligible and benefit from JMF’s grants.
